<p>It&#8217;s likely the WP nonce is invalid. Usually you get a message such as &#8220;Are you really sure you want to do this?&#8221; so I can understand why the blank page looks weird.</p>
<p>Have you modified the form at all? Perhaps you could paste the form&#8217;s markup into pastebin? I&#8217;ll take a look to see if I can see anything wrong</p>
