Default values shows up on Registration page

I’ve had this problem for a while now, for some reason, I don’t know why a name, date of birth, and interest information appear in their fields. Take a look: http://www.ivotepolls.com/register
For example: in Name, it says: Colin Heinink
Where is this problem originating?
I am using: BuddyPress 1.2.9
