Profile links not working – this is the first time I have witnessed this

On my WP/BP install (all current stable versions) the links tied to profiles seem to just flash and return to the index page. The links come up looking as such:
http://www.mydomain.com/members/Jerett%Patterson/
I deleted my install twice (both WP and BP) and I continue to get this issue. All plugins have been removed.
Any idea what is causing this and how to fix it permanently?
