Adding event to activity stream

I’m losing it here, what I’m trying to achieve must be possible.

I have forms on many pages of my site that allow users to post a review about the content on that page.

I would like….

1. User submits form
2. Activity stream entry created: USER has just submitted a form on PAGENAME (linked to page)

That’s it – but I can’t for the life of me figure it out. I’ve googled the heck out of this and I just can’t find any information on custom hooks that watch for form submissions.

Can anyone help?

What are you using to create the form for submitting a review?
The review creation process should have a hook.

Would that be the form id / submit button id?

The form was part of the theme I’m using.
