Contact form under Profile Tab

    I am looking for a plugin (or a way) to add a new tab on members’ profiles that would be called “Contact” so any logged in/off visitors could fill a contact form to contact admin regarding the member. Only issue I would find even if this existed would be to input the name of the member in the form.
    “Regarding: {Members full name}” or the url

    Does this exist? If not, how would I go about doing that?

    *OR*

    A way to append say cotact 7 forms to the bottom or end of the profile list on a member’s profile page.
