forms in the activity

I want to creat a social network, but too a ranking to athletes of Crossfit. For that I need to insert a form in the activity. In the form, I ask for the event done and the event appears in the cronology activity and too send to database to make the ranking.
My ask is, if there are a plugin do this or what hook I need to this whit a form y HTML.
