White screens of death are almost always PHP errors and as should such dump a message to the log file. I don’t know why it isn’t in this case.

The error I see immediately is that you haven’t put quote marks around the href or class parameters.

That’s definitely the problem. That error relates to the toolbar, which was first added in WP 3.2.x. Upgrade to 3.3 first!
