As far as I know, things should work fine with BP 1.2.5. I’ll do a bugfix release if needed, however. Welcome Pack 2.2 will probably be out in about two weeks, I’m focusing on my Achievements plugin at the moment (nearly beta!).

Not a bug; as you say, it depends on the state of the site/configuration when the activity was actually happened.

I’m not a core developer, but I suspect we’ll only release 1.2.6 if there is are show-stoppers or incompatibility with something in the 1.2.5 release.

Yes; the automatic hash generator has a rare bug, which you’ve found. This is on course to be fixed in BP 1.2.5.
