Is there a function I can add to bp-custom/functions.php that will always override this option to OFF position? Disable activity stream commenting on blog and forum posts: OPTION just keeps toggling on and off and I can’t think of any reason to ever use it. I’d hope in a future version it would be removed.

For the first issue, Usually when this happens (in custom themes) its because the achievements/single/home.php template is not the same as your other page customizations. Check either that file or the /members/single/plugins.php . Make sure those are all modified the same as your main /members/singles/home.php or else you’ll get stuff like this happen.

bp_is_blog_page() = your WP pages for the most part.

Hm I hope I didn’t point the wrong finger! I’ve disabled the W3TC on the secondary and on the main site and I’m still having issue.

I remember this happened once before during testing where the file path of the images was not using the main site domain instead it was using the secondaries.
