I created mine by using this service http://www.favicon.cc/ and uploading the .ico file to the root of my site and another copy within my theme folder. Seems to work =)

awesome! glad you like it =) once bp 1.3 is officially out in the wild, i’ll redo four of my most popular themes and get them up to snuff… maybe with google fonts? we’ll see. thanks for the message!

I use Gravity Forms to do this, although you could also look into TDO mini forms (https://wordpress.org/extend/plugins/tdo-mini-forms/) if you’re not keen on spending any money. I will say this… the GA plugin is *the* best plugin I’ve ever splurged on, bar none.
