Activity not pblishing for 1 specific user

We have a BP site and one member (and only that member) cannot publish activities, either from their profile or from /activity. The behavior is slightly different on each page.
From their profile, if they post an activity, it immediately appears at the top of the activity stream. But it is not visible at /activity, or the unique activity url (like /activity/p/1711/). When the profile is refreshed, the activity post is not there.
If we try posting and activity with a photo from the profile page, the activity text & photo immediately appears at the top of the activity stream. But it is not visible at /activity, or the unique activity url (like /activity/p/1711/). When the profile is refreshed, the activity post is not there. However, the photo does get uploaded and right clicking on it (when it’s still visible) and opening in a new tab works (ie the photo is visible at a url like /wp-content/uploads/2017/03/photo.jpg).
On /activity, the page refreshes after submitting an activity post, it says ‘Update Posted’, the page refreshed but the update never appears.
Why is this happening with one specific user?
