add custom field thumbnail to sitewide activity

Hi ,im trying to add a thumbnail of an image i have stored in custom field under each post under my activity field.
anyone can help me figure out where to do this and how ?
ive looked all over the net for an answer, all i found was ideas coming from 3 years ago..
please . little help 🙂
