Add exit survey to delete account page

I would like to add a survey for users to answer, if they intend to delete their account, to try and capture the reasons as to why they are leaving the site.

Firstly, can I just add this survey to the delete account page, and hide/show as necessary (probably tied in with the “I understand the consequences”)?

Secondly, what would be the best way to record the information, as it would be best saved to the database?

Wordpress version 4.5.4

Not easily accomplished without coding knowledge.

If I were developing this I would not force a user to fill in a form before deleting account. You could send an exit email with a custom email https://codex.buddypress.org/emails/custom-emails.
