Delete Account Tracking

Hi, I would like to add a required textbox on the delete account page so that I can capture why someone is leaving the site. Upon completion I would like it to send me an email with the account user’s username (if possible) and reason why he/she deleted his/her account. Help please.
Buddypress Version 2.7.4. WordPress 4.6.1 running Sweetdate Child theme.
