Duplicate members in widget and directory

As I found this issue is discussed before :

But left with no solution. I came to this problem recently as getting more members, so I hadn’t this problem before.

When a new user registers on site (it seems randomly) one of them appears in list with more than 30 times. (also in new members widget)

Site member directory (sort it on new members (کاربران تازه))

Notice that disabling plugins or same suggestions are not solutions.

This issue is nearly always a caching one, have you been running a cache plugin?

I use hyper cache and db cache reloaded plugin. If it’s cache related so by clearing whole cache it should get fixed, but that’s not.
