How to add maxmind database?

Hi i have 2 questions…
1. if i want to put countries , stat and city list in register form… and want to use maxmind database… how can i integrate?
2. After i make that if i want to put a search box the someone come and find person by country ,state ,city, age , blood group…
how can make like that…
all my focus is to make website for blood donors… and a register and search form…
