Issues with Activation post-signup

I’m having issues with users being unable to “activate” their account following a signup. They are sent an activation link via email without issue. However, upon clicking the activation link, they’re redirected to the homepage and the user remains in the “pending” category.
Does anyone know what may be causing this? Will provide more information if required.
