Start specific topic to start

I want to add list of options/categories to be selected for topic
For example : USERX started topic on mango
He has option to select type of mango.
He also has option to select origin of country.
So it would be like
Forum>Fruit
Topic>Mango
Category1>Alice (Over 50 different kind)
Category2>US (Over 200 Countries)
