Widgets must be located in /wp-content/blogs.dir/1/files/bpmh-widgets/ as you’re using wpmu .. Is that the case ?

I think Apache user does not have sufficient rights. Check if /wp-content/uploads/bpmh-widgets/ and /wp-content/uploads/bpmh-widgets-temp/ dirs exists, then check the rights of it.
