I’ve installed the plugin, enabled the login form and discovered that there are no checkboxes. You can check it on my site in sidebar
https://tbmods.ru or by screenshot http://prntscr.com/fticpm (“Forgot password?” string)

In registration form checkboxes are missing too.

How to solve this problem in my theme?

Using a browser inspection tool shows that the checkboxes are there but are not visible, probably due to your theme css.
To confirm, switch momentarily to a WP theme like 2016.

You’ll probably need to either change your theme css or add some custom css for BP.
