Error: username already being used

We’ve recently started having an intermittent issue with new user registrations where they fill out their registration form and upon submitting, get an error, “Username already being used”
They then iterate on the username and submit again, and continue to get the same error.
The problem is that the username is not already being used and even though they are getting an error, their request for registration has been received, so we end up with duplicate registration requests.
This error doesn’t seem to happen for everyone, which is making it harder to diagnose. I’m curious if anyone else has had this problem and has any insight into what is going on.
