groups and users on a network install

hello,
Im with many doubts about how users, groups, forums should/may be configured on a wordpress network install. I searched that some users are complaining about the fact that users created on one site become a user of all the sites on the network. However, I dont get that behavior. If a user is created on one site, he is not created on the other sites. How to accomplish this?
Relating to groups: Im only able to create groups from the network dashboard. The “Groups” item menu is not presented on the dashboard of the sites. Why?
What I would like to accomplish:
1- each site should have their own groups
2- each user should be created on all sites, so they can easily switch site.Can someone help achiving this?
