    BuddyPress Xprofile Custom Fields Type plugin is what I use to do this.

    It shows up on the profile as age. If you want it on the members page that takes some code editing. I saw a tutorial on BPDev site.

    If you click Activity on the admin menu you can filter by updated profile. That may help you some.

    I suggest Wordfence. It eliminates spam accounts from even signing up. It works great.

    Yes, it works with xProfile fields. You just set it as a text box. You can see it on my registration page. No need to join the site. Just type in the city box and it’ll populate.

    I would be interested in a plugin allowing a choice of cover images. Let us know how it turns out.

    I had an issue where some pages loaded but it showed as being signed out. I used this plugin. It’s worth a try.

    Remove HTTP is a plugin that automatically scans and removes both http: and https: protocols from all links. This helps resolve websites that are having “mixed content warnings” which is when the website has assets (images, JavaScript, and CSS) loading both HTTP and HTTPS.

    I think what you’re looking to do can be done with the Ultimate Member plugin. You would create a conditional field so if doctor were selected it would have separate field.

    There are quite a few social login plugins. I use WordPress Social Login. If you click Add New in plugins and search social login you’ll see quite a few.

    Are you using a theme? You can add some custom CSS to make your changes. Press F12 to view the elements in Chrome. Themeforest has Buddypress themes too that you might be interested in. You change the design as for your original question.

    I would suggest https://wordpress.org/plugins/buddypress-xprofile-custom-fields-type/ to create your profile fields. There also conditional field plugins that would allow you to set up different fields to members and doctors.

    I use Wordfence and CleanTalk and my spam is practically nothing. Very rarely does it let them through.

    You can add tabs on menus for friends, notifications, etc. Go to appearance – menu – buddypress and you can add links for signed in users wherever you want. You may need to click settings and tick Buddypress to get the options.
