Change to BP Default theme and check if the issue is corrected. If not, deactivate plugins (except BuddyPress) by batches to find the rogue plugin. What’s installed at domain root http://www.domain.com ?

@chamaxou basic troubleshooting includes changing to BP default theme and/or deactivating all plugins except BuddyPress to check what’s causing the conflict with your Add/Cancel friendship requests.

Re square avatars: have you tried holding down, CTL, SHIFT or Command key while pulling the top right corner of the crop tool?
