I have been testing this and playing with it in the past few days. It seems pretty damn good, if you don’t mind me saying so 
We have quite a few possible uses in mind and will be releasing some (very simple) plugins to build on the functionality a little once this becomes a stable release.
I haven’t had any problems similar to those you have mentioned. Have you double-checked the wp_bp_activity_user_activity_cached table to make sure that the activities you are ‘missing’ are properly timestamped and “hide_sitewide” isn’t set to 1?
