I got a similar problem.

When I hover My Account > Activity it says http://mydomain.com/members/admin/activity/. But almost always when I click it I end up at http://mydomain.com. the other times I end up at http://mydomain.com/members/admin/activity/ but with this error “Error 404 – Not Found”. I’m new to this and thought that I had done something wrong. But I have tried to uninstall, remove all files, db etc and re-installed. But it’s still the same.
