copy login form from line 40 to 52 of https://trac.buddypress.org/browser/tags/1.2.6/bp-themes/bp-default/sidebar.php
look at codes before and after those lines and see which other parts you need in your theme.
this is assuming you have the BP Template Pack installed and working with your WP theme.
